This book provides the first co-ordinated compilation of fault diagnosis and con� dition monitoring devices. It proceeds in three logical steps. The eady chapters deal with those conditions which contribute to deterioration and the consequent likely development of faults. The middle part of the book considers the various tech� niques of monitoring and discusses the criteria for their selection in different situ� ations. The final chapters provide a guide to the interpretation of the information signals deriving from monitoring, relating to reliability science and the mathematics of probability, and thus providing decision data on which management can act.

Vital and Timely

Breathtaking. Shatters any illusion that democracy is a given in the US or in any country. One detail among the sweeping history Snyder presents: his argument that the evisceration of privacy and the humiliation of the individual is a very old fascist technique. Consider that when you think about the email breaches of the last election, or why doxing is a weapon of choice among the cyber brown shirts. Another: the long and terrible legacy of dismantling the rule and protections of law as "exceptions" -- which quickly become permanent -- due to safety "emergencies." Americans may be tested on that sooner than later. Democracy has and can become totalitarianism in but a few months. The time to stand up for institutions, fair voting, and (this is surprisingly crucial) the ethics of your profession is NOW.

I had seen references to Timothy Snyder's writing, but none of the catalog was about a topic or period of interest. But, I saw that On Tyranny was on pre-order. It seemed interesting. I am so glad I ordered it, and so glad I read it. This book has the information that more of us NEED to read ... right now! Awesome book.

A must read: important, urgent --- and, sadly, necessary

Does Donald Trump mark “the tunnel at the end of the light” — is the United States on the verge of authoritarian rule? We’ve seen this meme on the Web since November 8. In the last few months, it’s become a larger conversation. But in writing by experts? Not to my knowledge.

In November, a piece headlined "Him: his election that November came as a surprise" read like a surprise. Because it sounded like recent history. The rallies. The racism. The conspiracy theories. Only toward the end do you get it — the year is 1933, the subject is Hitler.

A few weeks later, I was rocked by "20 Lessons from the 20th Century on How to Survive in Trump’s America," a Facebook post that had 12,000 “likes” and 15,000 “shares” — such a remarkable response it has been expanded and is now this book.

What was especially disturbing was the byline on both pieces. Timothy Snyder is Housum Professor of History at Yale, author of “Bloodlands: Europe Between Hitler and Stalin,” which received a dozen awards and has been translated into 33 languages. In 2015, Snyder published “Black Earth: The Holocaust as History and Warning,” now available in 24 foreign editions. Never has a subtitle been more accurate — or terrifying.

“On Tyranny" may be the most important civics book of the year. It’s certainly the most urgent. As Snyder said in a recent interview, “I think things have tightened up very fast; we have at most a year to defend the republic, perhaps less. What happens in the next few weeks is very important.”

Overwrought? Possibly. But I had relatives in Germany who, in 1933, felt that similar sentiments were overwrought. They “normalized” Hitler. I don’t have those relatives any more. So I called Timothy Snyder..

JESSE KORNBLUTH: Until I read “Bloodlands,” I knew almost nothing about what happened between 1930 and 1945 in Eastern Europe and the western Soviet Union, where Hitler and Stalin killed between 14 to 20 million people. In “Black Earth,” there was a fresh revelation: “Almost all of the Jews killed in the Holocaust lived beyond Germany.” Why were these revelations for me?
TIMOTHY SNYDER: We tend to think about history nationally, so we follow the experiences of German Jews, and once you focus on German national history, it’s hard to see others. I started with Jews and asked: Where were they killed?
JK: Why was the destruction of state institutions the critical factor in mass murder?
TS: Institutions are everything. For Germany to kill Jews, they had to make Jews unconnected to states. In Germany, that was a slow, legal process. So they started elsewhere, in places where for other reasons they destroyed the state.
JK: “Black Earth” suggests that Hitler backed into the Holocaust. He wanted Jews to be sent to Siberia or Madagascar. Why didn’t that happen?
TS: Hitler always wanted a world without Jews. In “Mein Kampf,” he said nature could only be restored by their elimination. He did not know just how this would take place. It was not clear to anyone, even to him, that Germans would shoot millions of Jews face to face. But when Germany invaded the Soviet Union, he gained a practical lesson in how mass murder is done.
JK: For Hitler, failure to conquer the Soviet Union and defeat Britain had to be caused by “enemies.” How did he come to think that and position himself as infallible?
TS: National Socialism fits into the larger Fascist tradition: pay no attention to facts, just follow the leader. Hitler said the Soviet Union would fall in 9 to 12 weeks. When that didn’t happen, he still had to be right, just, as it were, at a higher level. The cause of the defeat had to be the Jews.
JK: In some ways, Germany’s problem in the 1930s was about food shortages. These could have been solved by science — but science was suspect. Goebbels defined the purpose of a war of extermination as “a big breakfast, a big lunch, and a big dinner.” For German citizens, did the Holocaust really come down to this?
TS: It’s not quite that simple, but it’s important to understand that the Germans were living in a material world, and even a country as advanced as Germany had trouble with its food supply. Food was thus a matter of politics. Without a war to colonize Eastern Europe and, in particular, Ukraine for what Hitler called “Lebensraum,” there would have been no Holocaust.
JK: The thesis of “Black Earth” is that the Holocaust is “not only history, but warning.” In “Him” you’re especially scornful of lawyers and judges who submerged their professional ethics “in an
understanding of the greater good.” Why those people in particular?
TS: History doesn’t repeat itself. One person is not another. But the rule of law is a fundamental asset — it precedes democracy, it precedes the free market, everything depends on it. America is a very lawyerly society. Germans also cared a great deal about the rule of law. Yet in Germany, lawyers found ways to reverse the normal understanding of law. After that, anything could be done. So it might make sense for us to think about lawyers.
JK: In your Facebook post, you offer suggestions for good citizenship during the Trump presidency. You start with lessons learned from Nazi Germany: “Do not obey in advance. Defend an institution. Recall professional ethics.” Further down, it gets heavier: “Practice corporeal politics… Make new friends and march with them…. Make eye contact and small talk. This is not just polite. It is a way to stay in touch with your surroundings, break down unnecessary social barriers, and come to understand whom you should and should not trust. If we enter a culture of denunciation, you will want to know the psychological landscape of your daily life.” And then this: “Be as courageous as you can. If none of us is prepared to die for freedom, then all of us will die in unfreedom.” That is terrifying. “Die for freedom” — could it come to that here?
TS: It comes to that all the time. We’re just not used to it, because the exception is America in the last few generations. I don’t mean you should be prepared to throw yourself on the first available fire. I’m saying this: If none of us cares, we’ll all lose everything. Those are lessons from Fascism, Nazism but also Communism in the twentieth century. We have spent the last 25 years forgetting the history that we once thought vital to the preservation of our own institutions, convincing ourselves that our institutions will thrive on their own.
JK: In the last few months, we’ve seen scores of free-lance threats and acts of violence against Muslims. Do you see that as the precursor of an official assault on institutions?
TS: It’s a shot across the bow. Institutions are constantly under attack — they don’t stand up by themselves. What’s new in our country is a president who does not see himself as upholding an institution but as a person whose job is to denigrate institutions. We’re about to find out if all our big American talk about defending freedom is actually true.
JK: As a historian, do you believe that restricting freedom and targeting “enemies” start with an event: the Reichstag fire, for example, or 9/11? If that happens, what’s a useful response?
TS: Hitler used the Reichstag fire to suspend rights and declare a state of emergency that lasted for 12 years. When an event like that happens, It is natural that you experience fear and grief. But you have to understand your feelings can be exploited to undo a political order and resist. The Reichstag fire is a good example of that. A terrible thing, but compared to Nazi Germany….
JK: La Rochefoucauld wrote: “No one can look long at the sun or death.” At the end of 2016, I felt a general lessening of rage, a dull acceptance and dread — and a return to private concerns. Then came the protests. But commitment may wane again.
TS: You can pretend you have a choice. But you don’t have it. What I notice is a new interest in the 1930s among people in their 20s. We told them that history is over, go to work in finance. We were wrong. It’s crucial now how this generation reacts.
JK: Your final chapter addresses the ultimate defenders of decency — “the righteous few.” Who are they?
TS: That small minority who behaved heroically during the holocaust when all was black, when all institutions were destroyed. I worked hard in the Jewish sources to find and understand those rescuers. We tend to think we’re all like that. That’s na�ve. If government makes institutions go away, people will behave differently and in ways we now find unthinkable. In some future crisis, no doubt a few people will behave well. But we have no idea who they are now. It’s a mistake to count on being rescued. What we have to do is preserve the political order that we have come to take for granted.

Easy read for some otherwise difficult topics.

Interesting format for the topic. Stayed with the book to learn what happened to Mary! Even if most of the topics discussed are not relivent to you, the sections concerning estate planning and the identification of documents to be left for your heirs, is most helpful. I think everyone could find some useful information in this book while enjoying Mary's journey.

It breaks the mold of traditional organizational self-help books and uses the power of a story.

This book is quite unique in the sense that it breaks the mold of traditional organizational self-help books and uses the power of a story to motivate people on their journey to a more organized life.

Realizing the power of having someone to relate to, the authors decided to use a fictional character (Mary) who represents all the traits typically seen in chronically disorganized people. By doing this, the authors hope that they will be able to forge a stronger bond with the readers, show them that they are not the first ones to walk on the path to organization, and make them feel that their journey is easier to navigate if they do it with someone they trust.

Glad I didnt't read this book before my mother's back surgery

I wish I wouldn't have read this book. I say that not because the book was bad, but because it was too good. Some of the chapters just hit a little too close to home. One chapter in particular filled me with fear and another filled me with sorrow.

I started reading the book when my 80 year-old mother went in for major back surgery. I breezed through the first four chapters during her five hours of surgery and two hours in post-op care. The chapters were educational and enlightening. Dr. Welch makes a very compelling case about how we are being over-diagnosed and over-treated. The tone of the book was witty, so I was chucking and nodding my head as I read about data overload, U-shaped curves, the general uselessness of screening, the harm that false alarms can cause, the analogy of types of cancers to barn-yard animals: cancer that will never cause a problem are turtles, cancers that can be fought are rabbits, and cancers you can do nothing about are birds.

Then I got to Chapter 5 and the assumption: Action Is Always Better Than Inaction. First Dr. Welch gave some statistics on hospital infections after surgery: 1.7 Million "health care associated infections" associated with 98,987 deaths in 2002. Whoa doggie, my mom was in surgery. Next he talked about "postoperative cognitive dysfunction" after surgery particularly in the elderly. (Getting scare now - does 80 count as elderly?) Then he talked about needless surgery due to back pain, and how the majority of the time it doesn't work. I wanted to cry at this point, was mom doing this all for nothing? But I felt better when I read the statement: "Back surgery should only be done on patients who don't have back pain". My mom's surgery was to relieve nerve compression caused by severe scoliosis. But then the section on "invasive surgery" had me worried again: her cut was 15 inches long. And the section "Inaction = Allowing the Body to Heal" had me second guessing the decision to have the surgery. Talk about a roller coaster of emotions. I had to stop reading at this point.

I started reading again a few weeks later only to begin Chapter 7 and the assumption: It's all About Avoiding Death. The central theme of the chapter is that sometimes the quality of life is more important than prolonging life by a few months. Particularly with painful, debilitating cancer treatments. This chapter had me sobbing with tears pouring from my eyes. It brought back so many sad memories that are still raw and close to the surface. Three years ago my little brother was diagnosed with colon cancer that had moved to his liver. He fought the "cancer" battle for two years (MD Anderson). He went through all the pain and suffering because he wanted to see his son graduate from high school, he wanted to see his daughter go to her first dance, he wanted to go to the beach one last time. During one treatment (they inserted a tube through his groin and were pumping chemo drugs directly into his liver) he went into cardiac arrest. The doctors brought him back, but later he told me he wished they would have let him die. He said the treatments were too much for his wife, his kids, his family, and him; that sometime the quality of life is more important than quantity. He said it is better to quickly die with dignity than to wither away in excruciating pain. All my brothers' words were being echoed back to me by Dr. Welch.

I'm crying again. Excellent book but like I said I wish I had not read it.

Read it before it is too late

This book was especially relevant to me because my mother (65 years old) underwent a colonoscopy in which the doctor removed a few polyps and one of them had cancer cells close to the colon wall. Three out of four doctors said that she must have an operation which would remove a quarter of her colon (without saying anything about the risks of the operation). Only one doctor pointed out that may be all the cancer cells were taken out during the polyp removal, the risks and probability of success of the operation (which were horrible numbers). In the end my mother reasoned that the operation was not worth it. This was 6 months ago. Today she is alive and well, tending her garden.

Almost everybody has some sort of abnormality that won't hurt him but becomes a problem due to early diagnosis, i.e. you would be better of if did not perform the checks in the first place. Lesson: Don't do full checkups, only deal with things that you have symptoms of.

It shows the importance of randomization and biases that can be overlooked when performing medical trials, for example a trial based on volunteers can be biased because health conscious individuals are healthier than the average.

A wise doctor's prescripton for a happy, healthy, unmedicalized life

If I were president, the very first thing I would do is appoint Dr. Welch as US Surgeon General so he could slay the test pushing, pill popping, money grubbing healthcare monster. Today we are ordered to take tests we don't need for diseases we don't have and subjected to treatments that won't benefit us while we watch our wallets bled dry.

Unfortunately, I can't appoint Dr. Welch as Surgeon General, but I can recommend Less Medicine, More Health, a book that will teach you how to take control of your own healthcare so you can achieve the best health with a minimum of medical intrusion. Reading this book is like sitting down with a well respected, caring and wise physician, which is exactly what Dr. Welch is -- a professor of medicine and public health at Dartmouth Medical School, a doctor who was actually trained by the previous Surgeon General, Dr. C. Everett Koop.

I read Dr. Welch's previous book Overdiagnosis, and I have to say that while I loved the first book (and gave it a glowing review on amazon), the second book is even better because it doesn't just tell which medical tests to avoid, it tells you how to approach each interaction with doctors and the healthcare system, regardless of whether you are perfectly healthy, acutely ill, chronically ill, or dying. Moreover, it does it in an easy to read and enjoyable, conversational style.

I'm in my mid 60s and have given a lot of thought to how I want to live out the rest of my days. I'm happy to say it's exactly how Dr. Welch wants to live out the rest of his days. Like Dr. Welch, I want to stay as healthy as possible for as long as possible, but without medicalizing my life. That means I eat right, exercise, get vaccinations and treat only those conditions I actually have. I don't subject myself to anxiety provoking screenings to find new problems unless I have actual symptoms. I value my present happiness and ability to lead a pleasant, unmedicalized life far more than I do the small probability of extending my life by extraordinary means just so I can live long enough to develop another horrible illness. I want to enjoy a healthy, happy life today, but I absolutely refuse to making avoiding death my primary goal.

Thank you, Dr. Welch, for writing this important book and for spreading the message that we can safeguard our health by living right and seeking medical care only when necessary. We can take control and avoid being victims of the wasteful, harmful, greedy healthcare monster.
